Rally Waivers and I.C.E. Form

If you plan attend the VBA National Rally in Lake George, NY. please print and fill out the Rally Waiver.
Give the waiver to the rally leader upon arrival.

Although not required, it is recommended that you fill out an I.C.E Form and keep it on your person.
In case of an emergency it would provide medical personnel with needed information.
These are available from several places in different styles.
